Vip Perio Membership Plan

• 3 periodontal maintenance treatments
• 2 exams, oral cancer screenings and flouride treatments
• A set of routine x-rays yearly, and a panoramic or full mouth x-ray every 3 years
• An emergency exam, including any necessary x-rays
• 15% off additional treatment (fillings and sealants)
• 10% off additional treatment (major restorative work & periodontal treatment)

Available for adults ages 17 and over

Total plan valued at $1261!

This plan is available for patients who are not eligible for the adult membership plan due to exhibiting signs of periodontal disease, which is diagnosed by the dentist and hygienist.

Terms & conditions

• This is not insurance.
• A $50 enrollment fee per person applies to each individual signup.
• Prices are subject to change based on the specific dental needs of the patient.
• Services do not roll over into the following year. They must be rendered in between the sign up date and the expiration/renewal date.
• Services may be used only by the patient for whom the membership plan was purchased. Services may not be shared among family members, even if that individual also has his/her own membership plan. Each individual must use his/her own plan, and the benefits are attached to said plan.
• Membership plans are non-refundable.
• Each patient is responsible for renewing his/her own membership plan. Membership plans will not be auto renewed by Mountainview Dental.
• Membership plan fees are payable only in a lump sum, and may not be split into multiple payments.
• Membership plans may only be used at Mountainview Dental, located at 535 Route 52 Beacon, NY 12508.
• Membership plans may not be used in conjunction with insurance plans or other discount plans or financing options, such as Care Credit or Cherry.
