Many of our patients do not have dental insurance. Therefore, we have created this membership plan to help make quality dental care afforable to you. Being a member, you can enjoy these benefits that are readily available and facilitate your dental care needs.

*** There is a $139 Activation fee for all plan. Activation fee will cover your 1st month membership fee and will be paid one time annually. ***

Membership clubs are not insurance but payment arrangement provided by LV Dental for serivces rendered. Memberships are provided exclusively to uninsured patients of our practice and shall not be considered pre-payment for future services or payment for access to discounted services. Rather, our patient members are electing to make regular monthly subscription payments to preventative care instead of paying at the time of each service. Payment for any cosmetic and restorative treatment is due at the time of service. Membership dues must be current in order to receive services. Unpaid dues will require a reactivation annual fee of $139. The member is responsible for renewing membership annually to avoid reactivation fee. Membership club discounts may not combined with any other offer or discounts. It is solely the patient’s responsibility to schedule and keep their appointments. NO REFUNDS will be provided for dues paid under any circumstances, including failure to schedule and maintain appointments. Membership is non-transferable. A patient member’s monthly agreement is with the legal entity LV Dental. Monthly dues and fees for dental services subject to change at any time.
