Insurance Coverage:

Dr. De Ala accepts all major PPO plans, including but not limited to:

• Blue Cross/Blue Shield

We are also in network with Delta Premiere and MetLife. When we take your insurance information at the start of your appointment, we use that to submit claims on your behalf

Carecredit Financing:

Not every beneficial procedure in your treatment plan will be covered by insurance, and some of our patients may not have insurance at all. We offer CareCredit for patients in these circumstances. CareCredit is a financing program that allows you to pay off treatment plans over a series of small monthly payments.

Payment Forms Accepted:

We accept cash, check, Visa, MasterCard, and American Express in order to give you more options for covering the cost of treatment. You are responsible for any costs not covered by insurance at the time of treatment. This can include the full cost of service if your insurance plan rejects your claim within 90 days of treatment and the cost of treatment for any uninsured minors.
